But here’s something that might surprise you. Tooth pain isn’t always decay. Sometimes, it’s a tiny chip or hairline fracture in your tooth that you can’t even see in the mirror.

Cavity vs. Chip: Different Problems, Similar Pain
A cavity happens when bacteria eat through your enamel, creating a soft spot or hole. A chip or fracture, on the other hand, is a physical break in the tooth structure. Maybe you grind your teeth at night. Maybe you bit down too hard on an olive pit or an ice cube. Or maybe you have an old filling that has slowly weakened the surrounding tooth over time.
Both can hurt. Both can get worse if ignored. But they require completely different treatments.
The problem is, you can’t tell the difference on your own. And in a traditional dental office, the reflex might be to assume decay and start drilling right away.
How We Diagnose Without the Drill
At Dr. Dall’olmo’s practice, we slow down and listen, to you and to your tooth.
First, we ask questions. Does tooth pain hit only when you bite down and release? That often points to a chip or fracture. Does it linger with hot or cold? That could be decay reaching deeper layers.
Next, we use tools that don’t involve cutting into your tooth. We may use a special dye that clings to cracks. We may use transillumination; shining a bright light through the tooth to reveal hidden chips like a flashlight through a glass. Or we might even use a laser fluorescence device that measures density changes below the surface.
No shots. No numbing. And no drilling. Just information.
Why This Matters for Your Smile
Here is where Bio-Emulative Dentistry changes everything.
If we find a small chip, we might be able to smooth it out or bond it with a high-strength adhesive that actually seals the tooth back together. No removal of healthy structure. Just repair.
If we find early decay, we can sometimes reverse it with changes to your home care or apply mineralizing treatments. No filling needed at all.
But if we rush in and drill based on a guess? We risk turning a tiny problem into a much bigger one. That small chip gets prepped for a filling. That filling eventually weakens the tooth. And down the road, what could have been a simple bond turns into a much more involved procedure.
